What happens when I transition to post-16 with an EHC plan?

Transition in education is when you move into, between or out of educational settings for example from secondary school to college.

If you have an EHC plan you will begin to think about post-16 transition from year 9 onwards, with these discussions taking place during your annual reviews. This means that before you begin post-16 education or training you will have a transition plan in place to help support you in the next phase of your life.
